Thank you for your interest in CareSmart Illinois. We are dedicated to delivering quality educational programs and resources that support family caregivers, older adults, and adults with disabilities. To meet our mission, we provide the following services:
Community programs on topics such as dementia, user-friendly homes, fall prevention, caregiving, housing options, and self-advocacy
Information and referral
Telephone and in-person consultations on caregiving and aging issues
A comprehensive website of resources tailored to meet community needs
Caregiver and family learning/support networks, i.e., Care Cafes (in development)
CareSmart Illlinois' services are funded entirely by program sponsorships, grants, fundraising activities,donations, and charitable gifts from people like you.
To Make a One-Time Donation. You can make a gift by check or credit card (MasterCard, Visa, Discover or American Express).
To donate electronically via our website: Click on the Donate button above and follow the PayPal instructions to donate by credit card or check.
To donate by check via mail: Print a donation form, fill it out completely, and mail it (with your check) to our office at 3139 N. Augusta Dr., Wadsworth, IL 60083. Please make checks out to “CareSmart Illinois.”
